• All commercial insurance plans

Fees and Payments:
We make every effort to keep down the cost of your medical care and have extended the courtesy of accepting assignment on your insurance.  However the patient will be responsible for all deductibles and copays due at the time of service.  There will be a $10.00 processing fee for any additional claims or forms that must be completed by the physicians.  To avoid misunderstandings we advise you to discuss any financial difficulties with our office staff prior to services being rendered.
