Appointments - Contacts

Appointments:  Patients are seen on an appointment basis and we request that if you are unable to keep your appointment, please notify our office in advance.  Please bring your insurance card(s) and drivers license or some other form of identification.  

Telephone Calls:  Our staff has been properly trained to answer any questions that you may have and will relay your information to the doctor.  If your condition requires that you speak with the nurse or physician, calls will be returned at the earliest opportunity.

If you need assistance after office hours, the answering service will forward all messages and you call will be returned at he earliest opportunity.

Prescriptions and Renewals:  All prescriptions and authorizations for refills should be requested during regular office hours.
